Faculty, Presenter and Author Forms

 

All Faculty, Planning Committee Members, Presenters and Moderators of CUA-developed educational programs must complete these forms, which will be automatically submitted the CUA Office of Education.

All authors of CUAJ-accepted manuscripts must fill out the Conflict of Interest Disclosure form only.

All CUA Board Members and Committee Chairs must update their Conflict of Interest disclosures on a regular basis, specifically prior to Summer and Winter board meetings. In addition to industry relationships, please include intellectual disclosures, i.e. positions of leadership on other boards or health care organizations.
